Student Registration

Student registration is done by appointment only. At the high school level, registration takes more than one hour as placement testing is required. Elementary and Intermediate school registration takes approximately 1 hour. Please contact the school that your student will be attending to schedule an appointment. Parents who do not know which school their child will attend are invited to call: (570)839-7121 x30421 for the East Attendance Area and x31420 for the West Attendance Area.

We encourage registrations to be completed at least 2 weeks prior to the beginning of the school year. The age requirements for a kindergarten student is that the child must be 5 years old by September 1 and for a first grade student, the student must be 6 years old by September 1.

The first day of school for the 2008-2009 year will be September 2, 2008. This page lists documents required to register a student in the Pocono Mountain School District. A student may not be registered until ALL appropriate documents have been submitted.
Birth Certificate/Baptismal Record

If you are presenting a Baptismal Certificate, it must have an official raised seal.

The child's legal name will be used on all official documents. If these are not available, you must have proof that a replacement certificate has been requested.
Proof of Custody or Child Support

Natural Parent must have driver's license with current address, custodial arrangement affidavit if applicable, passport with birth date, I-94 card.

Child Support Statement verified by magistrate.

Divorced/separated parents must provide official document/court order which names the custodial parent.

Previous School Information

You must provide the exact address and phone number of the previous school, the child attended.

A report card, progress report, schedule, or letterhead from previous school may be substituted if it contains the school's address.

Record of Grades

A copy of the child's last report card from the previous school and/or transcripts must be presented.

Special Education

If the student receives Special Education services, a copy of all testing, reports, the student's IEP, and current paperwork must be provided when registering.

Immunization Records

Official immunization records from a doctor or the previous school. If these documents are not available, the student can still be registered, but will NOT be allowed to attend school.
Proof of Residency

Homeowner (Tax bill, settlement papers, occupancy permit, statement from realtor, utility bill)

Renter (Lease, letter from landlord with his/her telephone number, name, address, utility bill)

Note: A complete mailing address must be provided to the district.

Act 26 of 1995

This is a sworn statement to affirm/deny that a student has been suspended or expelled for violent behavior or a drug policy violation.
Social Security Numbers

Social Security numbers must be presented for the child and the parents/guardians.

Public Law 875

This document asks if the parents/guardians are living on Federal land, working in a Federal Facility, or part of the Armed Forces.
